# Feedback form template

A quick rating plus an open question. The email field is optional, so people can stay anonymous if they prefer.

1.  Get a free access key at [app.formdoc.dev/new](https://app.formdoc.dev/new).
2.  Replace `YOUR_ACCESS_KEY` with it and paste the form into your page.
3.  Rename or remove fields freely: each field’s `name` becomes its label in your email.

## Questions about this template

### Can people leave feedback anonymously?

Yes. The email field is optional. Without an email address you can still read the feedback, but you can’t reply.

### Can I change the rating options?

Yes. Edit the radio buttons. The value of the option someone picks is what appears in your email.
