# Contact form template

> Free HTML contact form template with name, email and message. Submissions are emailed to you, with spam protection. No backend code.

The classic three-field contact form. Short forms get more replies, so this one asks only for what you need to answer.

1.  Get a free access key at [app.formdoc.dev/new](https://app.formdoc.dev/new).
2.  Replace `YOUR_ACCESS_KEY` with it and paste the form into your page.
3.  Rename or remove fields freely: each field’s `name` becomes its label in your email.

## Questions about this template

### Can I add a phone number or company field?

Yes. Add another input with a name, for example \<input type="tel" name="phone">. Every field that has a name is sent, and its name becomes the label in your email.

### Where do the messages go?

To the email address you confirmed when you got your access key. Replying to the notification email replies straight to the visitor, because FormDoc sets the reply address to the email they entered.
