# Squarespace form to email

> Add a FormDoc form to Squarespace with a Code Block. Paste the HTML, add your key, and submissions are emailed to you.

In Squarespace, add the form as HTML with a **Code Block**.

**Plan:** which Squarespace plans allow Code Blocks and custom code has changed over time. Check Squarespace’s current plan details before you start.

1. Edit the page, add a block and choose **Code**.
2. Make sure the block is set to HTML, then paste the form and replace `YOUR_ACCESS_KEY` with your key from [app.formdoc.dev/new](https://app.formdoc.dev/new):

HTML

```html
<form action="https://api.formdoc.dev/v1/submit" method="POST">
  <input type="hidden" name="access_key" value="YOUR_ACCESS_KEY">
  <label for="fd-name">Name</label>
  <input type="text" id="fd-name" name="name" required>
  <label for="fd-email">Email</label>
  <input type="email" id="fd-email" name="email" required>
  <label for="fd-message">Message</label>
  <textarea id="fd-message" name="message" required></textarea>
  <input type="checkbox" name="botcheck" style="display:none" tabindex="-1" autocomplete="off">
  <button type="submit">Send</button>
</form>
```

3. Save, publish and send yourself a test.
